Home Health LPN - Licensed Practical Nurse
About The Position
Beacon Home Health is seeking a Licensed Practical Nurse (LPN) to provide quality patient care and skilled care in patients’ homes in Chattanooga, Tennessee, and surrounding communities. The LPN provides services within the scope of practice defined by state law, follows the plan of care, and coordinates with other members of the health care team. The role includes instructing patients’ families in patient care, maintaining a healthy environment for patients, and observing, recording, and reporting patients’ responses to treatment and changes in condition to the supervising nurse and/or physician daily. The LPN collaborates with physicians and RNs, documents care accurately and promptly, and does not alter the plan of care. The position requires commuting to multiple patient residences daily within the assigned geographic area. The employer is Beacon Home Health, an affiliate of The Pennant Group.
Requirements
- One year of clinical LPN experience.
- Current LPN license in the state where services are rendered.
- Current CPR certification.
- Driver’s license and vehicle insurance.
- Access to a dependable vehicle or public transportation.
- Ability to reliably commute to multiple patient residences daily within the assigned geographic area.
Responsibilities
- Provide quality patient care and skilled care in the patient’s home.
- Provide services within the scope of practice defined by state laws governing nursing and in accordance with the plan of care.
- Coordinate care with other members of the health care team.
- Instruct the patient’s family in the care of the patient and maintain a healthy environment for the patient.
- Observe, record, and report the patient’s response to treatment and changes in condition to the supervising nurse and/or physician daily.
- Collaborate closely with physicians and RNs.
- Document care accurately and promptly.
- Do not alter the plan of care.
- Commute reliably to multiple patient residences daily within the assigned geographic area.
