About The Position

Under the supervision of the Healthcare Coordinator of Day Habilitation Services, the Licensed Practical Nurse assists with coordinating medical services and provides healthcare services and medical documentation as needed. The position supports individuals in a day habilitation setting and works with recipients, their healthcare providers, residences, and the clinical team.

Requirements

• High school diploma or GED.
• Current registration with the Massachusetts Board of Registration on Nursing.
• One year of experience working with individuals with disabilities.
• One year of experience in a community setting.
• Knowledge of Day Habilitation Regulations and CARF standards.
• Knowledge of behavior management and human rights.
• Excellent organizational, problem-solving, and communication skills.
• Acceptable CORI results according to Bridgewell policy.

Responsibilities

• Assist the Healthcare Coordinator in coordinating medical services with each recipient’s primary care physician, medical clinic, or residence.
• Provide healthcare services to all individuals.
• Assist in completing required medical documentation, including seizure reports and medication and treatment charts.
• Establish and oversee groups in relevant areas, such as hygiene and women’s or men’s groups.
• Provide monthly in-services and training to staff regarding pertinent medical information, including services and communicable diseases.
• Inform the clinical team of medical changes, needs, and concerns.
• Complete medical treatments as indicated.
• Disburse medication and complete all required documentation.
• Purchase necessary medical supplies.
• Maintain appropriate certifications.
• Perform other duties as assigned.
