About The Position

The Licensed Practical Nurse (LPN) is responsible for providing healthcare services to individuals in an assigned residence, ensuring their health needs are met effectively and in a timely manner. This role involves direct patient care, monitoring health status, administering medications and treatments, and providing training to support staff. The LPN will also collaborate with physicians, nurse practitioners, and other service providers to ensure continuity of care and advocate for the individuals' needs. Additionally, the LPN will provide direct support services, including personal care, assistance with daily living activities, and crisis intervention, while maintaining a safe and therapeutic environment.

Requirements

  • Comprehensive knowledge of the observation and treatment of illness and disease.

  • Must demonstrate knowledge of, and willingness to adhere to, all departmental and agency policies and procedures.
  • Physical condition commensurate with demands of the job, including the unrestricted ability to assist in lifting and transferring on a regular basis.
  • New York State licensure or eligibility for New York State licensure to practice as a Licensed Practical Nurse.
  • Valid driver’s license and acceptable driving record, or equivalent accommodations required.

Nice To Haves

  • Professional experience preferred.
  • Experience working with individuals with disabilities preferred.
  • Current certification in Cardiopulmonary Resuscitation (CPR) and First Aid.

Responsibilities

  • Provides medical care as ordered by a physician or nurse practitioner, including skilled nursing services, tracheostomy care, and catheterization.
  • Administers medications and treatments as ordered.
  • Monitors the health status of individuals in the assigned residence.
  • Trains direct support professionals on general health-related issues, such as infection control, and specific individual health concerns.
  • Provides information to physicians/practitioners for medical appointments, schedules appointments, and may attend specialty consultations.
  • Documents pertinent contacts and new medical/health-related information.
  • Ensures documentation of vital signs, weight, intake/output, seizure activity, and response to medication/treatment.
  • Ensures medication and treatment records accurately reflect current physician orders, including ordering/filling prescriptions and maintaining adequate supplies.
  • Attends meetings to receive or provide medical/health-related information.
  • Communicates recommendations, orders, or changes in treatment plans from medical providers to the RN.
  • Communicates changes to residential staff per RN direction.
  • Maintains communication with Day Services and other service providers for continuity of health services and issue resolution.
  • Maintains a professional and cooperative relationship with community medical service providers.
  • Provides emergency medical treatment as needed.
  • Provides direct care and support services, including instruction and supervision of residents in personal care and daily living activities.
  • Assists individuals in pursuing personal interests in the community and at the residence.
  • Provides crisis intervention support services.
  • Intervenes and reports any incidences or unusual occurrences, ensuring immediate protective action is taken.
  • Ensures adequate diet according to identified needs and requirements.
  • Understands, identifies, and advocates for individual rights.
  • Models appropriate interaction and provides a positive role model for individuals.
  • Maintains compliance with State and Federal regulations and Agency policies.
  • Maintains an effective and positive relationship with family, correspondents, friends, service providers, and the community.
  • Reports pertinent information regarding individual needs or program operations to supervisors.
  • Participates in the Program Planning Team, including identifying, implementing, and documenting individual program plans.
  • Assists in providing a safe, clean, comfortable, and therapeutic environment.
  • Performs all other related duties as needed or assigned.
