About The Position

The Licensed Practical Nurse (LPN) is responsible for providing healthcare services necessary to meet the needs of individuals in an assigned residence. This role involves direct patient care, monitoring health status, administering medications and treatments, and providing training to support staff. The LPN also assists in the provision of personal care and direct support services, ensuring a safe, clean, and therapeutic environment for residents. This position may involve working in a certified residence as the sole LPN or as part of a team for residences with individuals who have high medical needs. The schedule can vary across first, second, or third shifts based on resident needs.

Requirements

  • New York State licensure or eligibility for New York State licensure to practice as a Licensed Practical Nurse.
  • Current certification in Cardiopulmonary Resuscitation (CPR) and First Aid.
  • Comprehensive knowledge of the observation and treatment of illness and disease.
  • Knowledge of medications and their prescribed usage.
  • Technical knowledge of medication administration principles.
  • Ability to work with families.

  • Valid driver’s license and acceptable driving record, according to guidelines established by the agency insurance carrier or equivalent accommodations required.

Responsibilities

  • Provides medical care as ordered by a physician or nurse practitioner, including skilled nursing services such as tracheostomy care and catheterization.
  • Administers medications and treatments as ordered by a physician or nurse practitioner.
  • Monitors the health status of individuals living in the assigned residence.
  • Provides training to direct support professionals on general health-related issues, including infection control and specific individual health concerns.
  • Provides information to the physician/practitioner for all medical appointments, schedules appointments, and may attend specialty and/or routine consults.
  • Documents pertinent contacts made on behalf of individuals and any new medical/health related information.
  • Ensures documentation of vital signs, weight, intake/output, seizure activity, and response to medication/treatment for assigned individuals.
  • Ensures medication and treatment records accurately reflect current physician orders, including ordering/filling prescriptions, ensuring over-the-counter medications are available and current, and maintaining stocked first aid equipment and supplies.
  • Attends meetings to receive or provide medical/health related information for individuals in the assigned residence.
  • Communicates recommendations, orders, or changes in the plan of treatment from medical providers to the RN and to residential staff as directed by the RN.
  • Maintains communication with Day Services and other service providers to ensure continuity of health services and timely resolution of health concerns.
  • Maintains a professional and cooperative relationship with community medical service providers.
  • Provides emergency medical treatment as needed.
  • Provides direct care and support services to residents, including instruction and supervision in personal care, daily living, and community activities.
  • Assists individuals in pursuing personal interests in the community and at the residence.
  • Provides crisis intervention support services to residents as needed or assigned.
  • Intervenes and reports any incidences or unusual occurrences as per policy/procedure, ensuring immediate protective action is taken.
  • Ensures adequate diet according to identified needs and/or requirements.
  • Understands, identifies, and advocates for individual rights.
  • Models appropriate interaction with residents and serves as a positive role model.
  • Maintains compliance with State and Federal regulations, and Agency program policies and procedures.
  • Maintains an effective and positive relationship with family, correspondents, friends, service providers, and the community.
  • Reports pertinent information regarding individual needs or program operations to supervisor(s).
  • Participates in the Program Planning Team, including identifying, implementing, and documenting individual program plans.
  • Assists in providing a safe, clean, comfortable, and therapeutic environment.
  • Performs all other related duties as needed or assigned.
