About The Position

The Licensed Practical Nurse provides nursing care to patients in an outpatient physician practice, following established standards and practices and the South Carolina Nurse Practice Act. Working under the direction of a practicing physician, APP, or RN, the LPN prepares patients for provider visits, updates medication histories at each visit, performs patient-related tasks as directed, and facilitates timely communication of clinical advice by phone or electronically. The LPN also ensures medical records are available to the care team before visits and coordinates with team members to provide exceptional patient service. Proficiency with HPP practice management software and eClinical Works EHR is expected. The position is based at Springhall Primary Care in Goose Creek, South Carolina, and is scheduled for 40 hours per week, Monday through Friday, 8:00 a.m. to 5:00 p.m. The role involves frequent patient and customer interaction, physical activity including lifting and moving patients, and exposure to conditions common to a healthcare environment. Call or weekend/holiday work may be required based on departmental needs.

Requirements

Graduate of an accredited school of practical nursing.
Current South Carolina LPN license, or a current compact/multistate LPN license in a recognized NCSBN Compact State if not a permanent resident of South Carolina.
Current American Heart Association BLS for Healthcare Provider Card.
Knowledge of organizational policies and procedures for providing patient care.
Strong patient-centered focus and effective verbal and written communication skills.
Ability to establish and maintain effective working relationships with patients, employees, the public, and other customers.
Ability to organize and manage competing priorities, solve problems, and exercise good judgment.
Knowledge of common safety hazards and precautions for establishing a safe work environment.
Skill in preparing and maintaining records.
Proficiency with HPP practice management software and eClinical Works EHR.

Responsibilities

Stage patients in preparation for provider visits.
Update patient medications in the medication history at each visit.
Perform patient-related tasks as directed by practice providers.
Facilitate timely communication of clinical advice to patients electronically or by phone, as directed by practice providers.
Ensure needed medical records are available to the care team and providers before patient office visits.
Coordinate with other team members to provide exceptional patient service.
Prepare and maintain records and help maintain a safe work environment.
