Licensed Practical Nurse | In-Home Skilled Nursing
About The Position
This role focuses on providing skilled nursing visits to patients in their homes, one patient at a time. The LPN Visit Nurse will travel to patients within a 35-mile radius of their home, assessing patient conditions, providing treatments, answering questions, educating families, and ensuring patients understand next steps. The position emphasizes the opportunity to focus on individual patient needs, observe, educate, communicate, and advocate while delivering necessary skilled nursing care. The goal is to help patients remain healthy, informed, independent, and safely at home.
Requirements
- Active Minnesota Licensed Practical Nurse (LPN) license
- Strong clinical observation and patient-care skills
- Excellent communication and organization
- Ability to work independently in patients' homes
- Compassionate and dependable approach to care
- Reliable vehicle, valid driver's license, and current auto insurance
- Ability to travel to patients within approximately a 35-mile radius of your home
Nice To Haves
- Home health experience is valuable, but not the only background that can prepare you for this role.
- If you've worked in: Skilled nursing, TCU or rehabilitation, Assisted living, Hospice, Clinic nursing, Other direct patient-care environments, you've already developed skills that can translate well into home health.
Responsibilities
- Skilled nursing observations and assessments
- Wound care and dressing changes
- Medication management and education
- Injections and ordered treatments
- Chronic disease monitoring
- Patient and caregiver education
- Reinforcing physician orders and plans of care
- Monitoring for changes in patient condition
- Communicating concerns to RN Case Managers and physicians
- Collaborating with therapists and other interdisciplinary clinicians
- Accurate and timely clinical documentation
