Licensed Practical Nurse | In-Home Skilled Nursing
About The Position
This role is for a Licensed Practical Nurse (LPN) who will conduct home health visits, focusing on one patient at a time. The LPN will travel to patients within a 35-mile radius of their home to provide scheduled skilled nursing visits. The position emphasizes providing focused, individualized patient care, including assessments, treatments, education, and monitoring. The goal is to help patients remain healthy, informed, independent, and safely at home. The role offers the opportunity to practice independently in the field, build ongoing relationships with patients and caregivers, and utilize nursing judgment and observation skills.
Requirements
- Active Minnesota Licensed Practical Nurse (LPN) license.
- Strong clinical observation and patient-care skills.
- Excellent communication and organization.
- Ability to work independently in patients' homes.
- Compassionate and dependable approach to care.
- Reliable vehicle, valid driver's license, and current auto insurance.
- Ability to travel to patients within approximately a 35-mile radius of your home.
Nice To Haves
- Home health experience is preferred.
- Experience in Skilled Nursing TCU or rehabilitation.
- Experience in Assisted Living.
- Experience in Hospice.
- Experience in Clinic nursing.
- Experience in other direct patient-care environments.
Responsibilities
- Conduct skilled nursing observations and assessments.
- Provide wound care and dressing changes.
- Manage and educate patients on medications.
- Administer injections and perform ordered treatments.
- Monitor chronic diseases.
- Educate patients and caregivers.
- Reinforce physician orders and plans of care.
- Monitor for changes in patient condition.
- Communicate concerns to RN Case Managers and physicians.
- Collaborate with therapists and other interdisciplinary clinicians.
- Complete accurate and timely clinical documentation.
