We are seeking a full-time Licensed Practical Nurse (LPN) to provide clinical services to all clients receiving ACT team services, which includes medication monitoring and documentation; delivering psychopharmacologic and medical services within the scope of practice of the individual’s license; service coordination, including working with the ACT team to monitor each individual’s clinical status and response to treatment; crisis evaluation and intervention; symptom evaluation and management; individual counseling and psychotherapy; substance abuse treatment; work-related services; activities of daily living services; social, interpersonal relationship, and leisure-time activity services; support services or direct assistance to ensure that clients obtain the basic necessities of daily life; and education, support, and consultation to individuals’ families and other major supports. Monitor and oversee all medical/clinical aspects of services delivered. Provide medication monitoring and documentation. Deliver psychopharmacological and medical services within your scope of practice. Collaborate with ACT team members to monitor clinical status and response to treatment. Conduct crisis assessments, symptom management, and intervention planning. Provide individual counseling, substance abuse treatment, and psycho-educational support. Assist in creating transition plans when clients move in and out of services. This description outlines the general scope and level of work for this role.
