About The Position
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide direct nursing care in a Long-Term Care setting. The ideal candidate will have strong clinical skills, experience in resident-centered care, medication administration, and the ability to work collaboratively with residents, families, and interdisciplinary healthcare teams.
Requirements
- Graduate of an accredited LPN program.
- Active Massachusetts LPN License.
- Current BLS Certification.
- CPR and AED Certification.
- Strong clinical, documentation, and communication skills.
- Ability to work every other weekend and rotating holidays.
Nice To Haves
- LTC, SNF, Rehab, or Geriatric Nursing experience preferred.
- Experience supervising Nursing Assistants preferred.
Responsibilities
- Provide direct nursing care according to individualized care plans and physician orders.
- Administer medications, treatments, and therapeutic interventions safely and accurately.
- Perform wound care, catheter care, tube feedings, specimen collection, and other nursing procedures.
- Monitor resident conditions and report changes to physicians and care teams.
- Record vital signs and maintain accurate clinical documentation.
- Implement physician orders and update resident records.
- Supervise Nursing Assistants and coordinate resident care activities.
- Educate residents and families regarding treatments and ongoing care needs.
- Ensure compliance with LTC regulations and facility policies.
- Maintain a safe and therapeutic care environment.
