About The Position

Reporting to the Patient Care Manager or Resident Care Manager, the Household Licensed Practical Nurse (LPN) provides compassionate, resident-centred nursing care as part of an interdisciplinary household team. Working within a client- and family-centred model of care, the Household LPN assesses, plans, implements, and evaluates care while supporting residents' physical, social, spiritual, and psychological well-being. In this role, you will collaborate closely with residents, families, and healthcare team members to develop care plans, advocate for residents, and ensure individual preferences, abilities, and routines are reflected in daily care. The Household LPN contributes to a safe, respectful, and home-like environment, participates in care conferences and team meetings, supports the orientation of new staff and students, and works with the household team to promote the highest quality of life for residents. Requirements

  • Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience
  • Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant
  • Current basic cardiac life support certification
  • Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service
  • Physical ability to carry out the duties of the position
  • Ability to apply decision making and analytical skills
  • Ability to role model conflict resolution strategies
  • Ability to prioritize and organize work
  • Ability to teach and identify patient/resident learning needs

Responsibilities

  • Provide safe and respectful nursing care, adhering to established policies, individualized care plans, and priorities
  • Conduct thorough assessments, monitor signs/symptoms, and communicate with the RN/RPN as needed
  • Prepare patients for procedures, transfers, and admissions, coordinating with the interdisciplinary team
  • Administer medications per protocol, monitor effects, and maintain communication with the team
  • Utilize evidence-based resources to stay updated on nursing practices
  • Respond to incidents such as codes, injuries, spills, and alarms, following procedures
  • Assist with staff orientation, demonstrating procedures and providing support
  • Address patient/resident and family learning needs, provide educational materials, and make referrals
  • Attend care planning meetings, provide input, and advocate for patients/residents
  • Collaborate with the team to develop and evaluate individualized care plans
  • Assist with discharge planning based on patient status and external supports
  • Operate and clean equipment, report malfunctions, and handle hazardous materials
  • Perform clerical duties such as answering phones, faxing, and maintaining records
