Reporting to the Patient Care Manager or Resident Care Manager, the Household Licensed Practical Nurse (LPN) provides compassionate, resident-centred nursing care as part of an interdisciplinary household team. Working within a client- and family-centred model of care, the Household LPN assesses, plans, implements, and evaluates care while supporting residents' physical, social, spiritual, and psychological well-being. In this role, you will collaborate closely with residents, families, and healthcare team members to develop care plans, advocate for residents, and ensure individual preferences, abilities, and routines are reflected in daily care. The Household LPN contributes to a safe, respectful, and home-like environment, participates in care conferences and team meetings, supports the orientation of new staff and students, and works with the household team to promote the highest quality of life for residents.
