About The Position

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join a long-term care team dedicated to providing exceptional resident-centered care. The LPN will be responsible for delivering direct nursing care, administering medications and treatments, monitoring resident health status, and collaborating with interdisciplinary healthcare professionals to promote optimal outcomes and quality of life.

Requirements

  • Graduate of an accredited Practical Nursing program required.
  • Active Licensed Practical Nurse (LPN) license in the State of Pennsylvania required.
  • Current CPR/BLS certification preferred.

  • Previous experience in Long-Term Care, Skilled Nursing, Assisted Living, Rehabilitation, or Geriatric Care preferred.

Responsibilities

  • Provide direct nursing care to residents in accordance with individualized care plans.
  • Administer medications, treatments, and nursing interventions safely and accurately.
  • Monitor, assess, and document residents' vital signs, symptoms, and overall health status.
  • Observe and promptly report changes in resident conditions to the Registered Nurse and/or physician.
  • Assist with resident admissions, transfers, and discharges.
  • Maintain accurate, timely, and complete nursing documentation.
  • Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate resident care.
  • Educate residents and family members regarding medications, treatments, and care plans.
  • Respond appropriately to resident needs, emergencies, and changes in condition.
  • Ensure compliance with facility policies, nursing standards, and applicable regulatory requirements.
  • Promote resident safety, dignity, comfort, and quality of life.
  • Participate in care conferences, team meetings, and quality improvement initiatives.
  • Support a positive, respectful, and team-oriented care environment.
