About The Position

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality nursing care within a long-term care and skilled nursing environment. The ideal candidate will work collaboratively with residents, families, physicians, and interdisciplinary team members to ensure exceptional patient care, promote resident well-being, and maintain a safe and supportive healthcare environment.

Requirements

  • Graduate of an accredited Practical Nursing program required.
  • Active Licensed Practical Nurse (LPN) License in the State of Kansas required.
  • Ability to work night shifts and rotating schedules.

Nice To Haves

  • Current CPR/BLS Certification preferred.
  • Previous Long-Term Care, Skilled Nursing Facility (SNF), Rehabilitation, Assisted Living, or Geriatric Care experience preferred.
  • Strong clinical nursing and medication administration skills.
  • Excellent patient care and resident advocacy abilities.
  • Strong communication and interpersonal skills.
  • Ability to work effectively within an interdisciplinary healthcare team.

Responsibilities

  • Provide direct nursing care and treatments in accordance with individualized care plans.
  • Administer medications and treatments safely, accurately, and in compliance with established protocols.
  • Monitor resident health status and promptly report changes in condition to the healthcare team.
  • Perform ongoing assessments and assist in developing and updating care plans.
  • Document nursing assessments, interventions, treatments, and resident responses accurately and timely.
  • Assist with resident admissions, transfers, and discharge processes.
  • Promote resident comfort, dignity, independence, and quality of life.
  • Collaborate with physicians, registered nurses, therapists, and support staff to coordinate comprehensive care.
  • Respond appropriately to emergencies and provide nursing interventions as required.
  • Educate residents and family members regarding care plans, treatments, and health-related concerns.
  • Ensure compliance with facility policies, nursing standards, and applicable regulatory requirements.
  • Participate in care conferences, staff meetings, and quality improvement initiatives.
  • Maintain a safe, clean, and resident-centered care environment.
