About The Position

Benchmark at Rye is seeking a compassionate Licensed Practical Nurse (LPN) to deliver nursing care to residents in a warm, comfortable, home-like environment. The LPN works with a team of associates and helps support residents’ independence while coordinating their care. This is a per diem opportunity available on all shifts, with compensation of $36–$44 per hour.

Requirements

• Current state LPN license and CPR certification.
• Graduate of an approved LPN program, as required by state requirements.
• One to two years of experience working as a nurse is preferred.
• Previous paid or volunteer experience working with elderly people is highly desirable.

Responsibilities

• Accurately assess, communicate, and document residents’ status.
• Observe and report significant changes in residents’ behavior and health to the Resident Care Director, physician, family, RCAs, and Executive Director in accordance with state regulations.
• Direct and supervise Resident Care Assistants’ daily work assignments.
• Mentor, guide, and direct Lead Resident Care Assistants in all aspects of their work.
• Coordinate care needs with community providers through an effective case management process.
• Balance service demands with support for resident independence.
• Recognize and help prevent elder abuse, neglect, and exploitation, and report concerns to appropriate sources in accordance with state regulations.
• Respond to emergencies and personal emergency response system alerts, and determine when to call for backup.
