Licensed Practical Nurse (LPN) - Per Diem Shifts
About The Position
Alvita Care is seeking compassionate, reliable, and professional Licensed Practical Nurses (LPNs) for immediate full-time floor staffing opportunities in Patchogue, NY. We are looking for dedicated nurses who are committed to providing high-quality resident care in a facility setting. Available Shifts Primary Shifts: Most available shifts are 3 PM–11 PM , on weekdays and weekends. Employment Options: Full-time or part-time hours available. Per-Diem Positions: All positions are per diem, with scheduling based on facility staffing needs.
Requirements
- Valid New York State LPN license required.
- Must be authorized to work in the United States.
- Must have updated medical and vaccination records or be willing to obtain them.
- Must complete the required drug screening or habituation form.
- Must successfully complete Alvita Care's onboarding process before starting the assignment.
- Must be able to meet the facility's scheduling and staffing requirements.
Nice To Haves
- Previous nursing experience preferred.
- Long-term care or facility experience preferred.
- Reliable, professional, and patient-centered.
- Strong communication and documentation skills.
Responsibilities
- Provide quality nursing care and support to residents.
- Administer medications and treatments within the scope of the LPN license.
- Monitor residents' conditions and report changes to the appropriate healthcare professionals.
- Maintain accurate and timely documentation using the facility's charting system.
- Follow facility policies, procedures, and infection control protocols.
- Collaborate with other healthcare professionals to promote resident safety and well-being.
- Assist with other nursing duties as assigned within the scope of practice.
