About The Position

The Home Health Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) works on a per diem basis under the supervision of a Registered Nurse to provide skilled nursing care to patients in their homes. The role involves coordinating with the RN and physician, supporting patients and caregivers with self-care, monitoring patient health, managing medications, carrying out physician-ordered procedures, and documenting care. The position offers a compensation structure that includes per-visit-point pay for visits and hourly pay for certain non-visit activities, such as orientation. Visits are assigned according to patient and business needs, and weekly visit volume may vary. Requirements

At least 1 year of LPN or LVN experience.
Current LPN license specific to the state(s) assigned for work.
Current CPR certification.
Valid driver’s license, reliable transportation, and liability insurance.
Pass a drug test before beginning employment.

Responsibilities

Verify the plan of care with the RN before each visit.
Collaborate frequently with the physician and RN to ensure patient needs are met, and notify them as soon as possible of changes in a patient’s condition.
Teach patients and caregivers self-care techniques.
Check vital signs.
Review all medications and perform medication reconciliation at each visit.
Perform physician-ordered specialized procedures, such as Foley catheter changes.
Prepare equipment and materials for treatments using aseptic technique.
Complete documentation in a timely and accurate manner.
