About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Home Health Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) works under the supervision of a Registered Nurse to provide skilled nursing care to patients in their homes. The role is PRN. Compensation includes per-visit-point pay for patient visits and hourly pay for certain non-visit activities. The stated annual total cash target assumes full-time employment at full productivity and is not a statement that this PRN role is full-time. The role offers performance recognition and development opportunities. Benefits and incentives are subject to eligibility requirements. Candidates must pass a drug test before beginning employment.

Requirements

Current LPN license specific to the state or states where assigned to work.
Current CPR certification.
At least 1 year of LPN or LVN experience.
Valid driver's license, reliable transportation, and liability insurance.
Pass a drug test before beginning employment.

Responsibilities

Verify the plan of care with the RN before each visit.
Collaborate frequently with the physician and RN to ensure patient needs are met, and notify them as soon as possible when a patient's condition changes.
Teach patients and caregivers self-care techniques.
Check vital signs.
Review all medications and perform medication reconciliation at each visit.
Perform specialized procedures ordered by the physician, such as Foley catheter changes.
Prepare equipment and materials for treatments using aseptic technique.
Complete documentation accurately and on time.
