About The Position

Sentara Medical Group is seeking Licensed Practical Nurses (LPNs) for full-time and part-time positions across various locations including Chesapeake, Portsmouth, Norfolk, Suffolk, Virginia Beach, and surrounding Southside communities. This role involves supporting providers and registered nurses in direct patient care within an ambulatory setting. Responsibilities include rooming patients, gathering health information, measuring vital signs, administering medications and immunizations, collecting specimens, and performing simple diagnostic tests. The LPN will also prepare patients and rooms for examinations, assist with procedures, and provide patient education and follow-up instructions. Additionally, the role supports patient flow, manages messages and refill requests under provider direction, and may guide the work of medical assistants and aides. Accurate documentation in the electronic health record is crucial for care coordination and safety.

Requirements

  • Completion of an accredited practical nursing program leading to LPN/LVN eligibility is required.
  • A high school diploma or equivalent is required.
  • Current, unencumbered state Licensed Practical Nurse (LPN/LVN) license is required.
  • Successful completion of the NCLEX-PN examination is required.
  • Basic Life Support (BLS) certification is required.

Nice To Haves

  • Experience in an ambulatory or physician practice setting is preferred.
  • One to two years of nursing experience is preferred, ideally in an ambulatory or outpatient setting.
  • New graduates of an accredited practical nursing program may be considered.

Responsibilities

  • Assists providers and registered nurses in delivering direct patient care in the clinic setting.
  • Rooms patients, measures vital signs, and records health information in the electronic health record.
  • Administers medications and immunizations within the LPN scope of practice.
  • Collects blood specimens and performs simple diagnostic tests as ordered.
  • Prepares patients and treatment rooms and assists with examinations and procedures.
  • Provides patient education and follow-up instructions within scope.
  • Supports patient flow and processes messages and refill requests under provider direction.
  • Directs the day-to-day work of medical assistants and aides as assigned.
  • Follows infection prevention and safety standards consistent with DNV and CMS requirements.
  • Performs other related duties as assigned.
