About The Position

Ambercare Homecare is seeking a Licensed Practical Nurse to join its growing team in Albuquerque, New Mexico. This PRN position covers 2–3 days in the East Mountains. The nurse provides direct patient care under the state Nurse Practice Act, carries out the registered nurse’s plan of care, documents care accurately and promptly, and educates patients and caregivers. The role also involves coordinating home health services, using equipment and supplies safely and efficiently, and contributing to professional development.

Requirements

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years of nursing experience.
• Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
• Maintain valid CPR certification.
• Reliable transportation, a current driver’s license, and appropriate automobile insurance.

Nice To Haves

• Community or home health experience.
• Medical/surgical experience.

Responsibilities

• Provide direct patient care as defined by the state Nurse Practice Act.
• Implement the plan of care initiated by the registered nurse.
• Document care accurately and promptly in accordance with the plan of care.
• Assess patients and educate patients and family or caregivers about self-care and information related to the diagnosis and plan of care.
• Coordinate home health services and report identified needs for other disciplines to the registered nurse and/or Clinical Supervisor.
• Use and prepare equipment and supplies effectively and efficiently while adhering to aseptic technique.
• Participate in personal and professional growth and development.
• Perform other duties assigned by the registered nurse.
• Provide health care instructions to patients as appropriate to the assessment and plan of care.
