About The Position

Benchmark Senior Living is seeking a compassionate Licensed Practical Nurse (LPN) to provide nursing care to residents in a warm, comfortable, home-like environment. The role is part of a team of associates who support one another and build relationships with residents. The position is suited to someone dedicated to assisting others and proud of their work. The LPN assesses, communicates, and documents residents’ status; reports significant changes in behavior or health; and supervises Resident Care Assistants. The position also coordinates care with community providers, supports resident independence, helps prevent and report elder abuse, neglect, and exploitation, and responds to emergencies.

Requirements

Current state LPN license and CPR certification.
Graduate of an approved LPN program, per state requirements.

Nice To Haves

One to two years of experience working as a nurse is preferred.
Previous experience working with elderly people in a paid or volunteer position is highly desirable.

Responsibilities

Accurately assess, communicate, and document residents’ status.
Observe and report significant changes in residents’ behavior and health to the Resident Care Director, physician, family, RCAs, and Executive Director, in accordance with state regulations.
Direct and supervise the daily work assignments of Resident Care Assistants.
Mentor, guide, and direct Lead Resident Care Assistants in all aspects of their work.
Coordinate care needs with community providers through an effective case management process.
Balance service demands with support for resident independence.
Recognize and help prevent elder abuse, neglect, and exploitation, including financial exploitation, and report concerns to appropriate sources per state regulations.
Respond to emergencies and personal emergency response system alerts, and know when to call for backup.
