About The Position

Benchmark is seeking a compassionate Licensed Practical Nurse to deliver nursing care to residents in a warm, comfortable, home-like environment. The role is part of a team focused on supporting residents and building lasting relationships with residents and associates.

Requirements

• Current state LPN license and CPR certification.
• Graduate of an approved LPN program, per state requirements.

Nice To Haves

• One to two years of experience working as a nurse is preferred.
• Previous experience working with older adults in a paid or volunteer position is highly desirable.

Responsibilities

• Accurately assess, communicate, and document residents’ status.
• Observe and report significant changes in residents’ behavior and health to the Resident Care Director, physician, family, RCAs, and Executive Director, as required by state regulations.
• Direct and supervise Resident Care Assistants’ daily work assignments.
• Mentor, guide, and direct Lead Resident Care Assistants in all aspects of their jobs.
• Coordinate residents’ care needs with community providers through an effective case management process.
• Balance service demands while supporting resident independence.
• Recognize and help prevent elder abuse, neglect, and exploitation, including financial exploitation, and report concerns to appropriate sources as required by state regulations.
• Respond to emergencies and personal emergency response systems, and determine when to call for backup.
