About The Position
Advantage Home Health Services is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide home health care in Lancaster, Pennsylvania, and nearby areas. The role offers one-on-one patient care in patients’ homes, flexible scheduling, support from a collaborative clinical team, and the potential use of a company car for visits. The nurse delivers care according to personalized patient care plans and works with other clinicians to support quality care.
Requirements
- Active LPN license in Pennsylvania.
- Passion for home health and patient-centered care.
- Strong communication and time management skills.
- Ability to work independently while collaborating with a supportive team.
Responsibilities
- Deliver nursing care as outlined in each patient’s personalized care plan.
- Partner with RNs, therapists, and other clinicians to ensure top-quality care.
- Monitor and accurately report patient conditions and changes.
- Build meaningful relationships with patients and their families.
- Provide comfort, dignity, and hope during every visit.
