About The Position

As a CenterWell Home Health nurse reporting to the Clinical Manager, the Home Health Licensed Practical Nurse provides personalized, one-on-one care to patients in their homes to help them regain independence. The LPN works with physicians and clinicians and provides skilled nursing care under the direction of an RN. The role includes implementing individualized care plans, educating patients and families, maintaining accurate care records, communicating changes in patient status, and participating in care coordination and discharge planning. The position is full-time, with 40 scheduled hours per week.

Requirements

• Current nursing license in the practicing state.
• Valid driver's license, auto insurance, and reliable transportation.
• Current CPR certification.
• 2 years of experience as an LPN/LVN in a clinical setting.

Nice To Haves

• Nursing experience in a Home Health or Hospice setting.

Responsibilities

• Provide skilled nursing care to patients in their homes under the direction of an RN.
• Implement individualized care plans in collaboration with the patient, family, and healthcare team.
• Educate patients and their families on disease management, treatment options, and self-care techniques.
• Maintain accurate records of patient care, including documentation of visits, interventions, and outcomes.
• Promptly communicate with patients, physicians, clinical managers, and other healthcare team members regarding patient or family needs and status changes.
• Participate in care coordination activities and discharge planning.
