About The Position
A compassionate and reliable Licensed Practical Nurse (LPN) is needed to provide high-quality care in a long-term care/skilled nursing setting. This role involves supporting patient assessments, administering medications, monitoring resident health, and assisting in the implementation of individualized care plans to ensure comfort and safety.
Requirements
- Active North Dakota LPN License
- IV Certification
- Basic Life Support (BLS) Certification
- Ability to work rotating night shifts and weekends
Nice To Haves
- Experience in Long-Term Care (LTC) or Skilled Nursing facilities
- Familiarity with Matrix EMR system
Responsibilities
- Provide direct resident care in a long-term care environment
- Assist with patient assessments and monitor health status
- Administer medications and IV therapy as prescribed
- Maintain accurate and timely clinical documentation using EMR systems (Matrix)
- Support and implement individualized care plans
- Monitor and report changes in patient condition to the care team
- Ensure adherence to patient safety, infection control, and facility protocols
- Collaborate with interdisciplinary teams to deliver quality care
