About The Position
Commonwealth Home Health Network is looking for Licensed Practical Nurses to join our team. CHHN provides skilled and unskilled services to seniors, older adults, and those who are disabled. Utilize the nursing process to assess, plan, implement, and evaluate patient care. On each assignment, provides skilled nursing care/services in accordance with prescribed orders Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition. Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.
Requirements
- Valid nursing license (required)
- 1+ years of licensed practical nursing experience
- Associate degree in nursing (required)
Nice To Haves
- Home health experience is a plus.
