About The Position

PACE, a member of Empath Health is seeking a compassionate Licensed Practical Nurse (LPN) to join our interdisciplinary PACE team in Tampa, FL. In this role, the Licensed Practical Nurse will assist in providing care in the clinic setting as well as in the home. The Program of All-Inclusive Care for the Elderly (PACE) is a unique care model designed to help older adults remain safe and independent in their own homes. It offers Relationship-Based Care with Real Impact, allowing you to build long-term connections with participants, supporting them through all stages of care. It is a meaningful alternative to traditional inpatient or facility-based care. It also features a Team-Driven, Preventive Focus, where you will collaborate closely with physicians, social workers, therapists, and other clinicians in a proactive care model focused on prevention, wellness, and holistic well-being.

Requirements

  • Graduate of a school of professional nursing program required.
  • Current Florida LPN License
  • Valid CPR certification from the American Red Cross or American Heart Association.
  • A minimum of one year’s experience in working with the frail or elderly population required.

Responsibilities

  • Take responsibility for evaluating and improving the quality of participant care and services provided.
  • Document in the medical record, to include significant changes in participant status.
  • Update care plans with changes in participant status.
  • Complete documentation of all visits (including episodic, routine, phone calls, etc) against the corresponding care plan elements.
  • Review medical record to ensure that appropriate information such as labs, specialist reports and diagnostic test results are captured in the medical record.
  • Identify participant’s overt problems/needs and set priorities.
  • Collaborate with the RN on a plan of care based on nursing process which incorporates the plans of other disciplines and continuing care needs.
  • Ensure care provided conforms to accepted practice standards; provides correct treatments/procedures and other care as prescribed and according to participant care standards.
