About The Position

This position will be supporting PACE participants in our Laurelhurst site. Providence PACE is a Program of All-Inclusive Care for the Elderly that strives to keep older adults as healthy as possible living in the community through clinics, home visits and more. Join our team to help empower elders in your community to live active, independent lives.

Requirements

  • Bachelor’s Degree in Nutrition, Food and Nutrition and/or Dietetics (Educational requirements as established by the American Dietetic Association).
  • Upon hire: National CDR Registered Dietitian - Commission on Dietetic Registration
  • Within 6 months (180 days) of hire: Oregon Dietitian License

Nice To Haves

  • Master's Degree in Nutrition, Food and Nutrition, Dietetics or other related field
  • 1 year experience in clinical setting for generalist.

Responsibilities

  • Provides nutrition assessment, counseling, and planning upon inter-disciplinary team (IDT) referral for participants identified as being at nutritional risk according to established screening protocol or as ordered by physician.
  • Monitors and documents the participant's response to mode of nutritional therapy used.
