About The Position

Vanderbilt University Medical Center (VUMC) is seeking a Licensed Practical Nurse (LPN) to join their Primary Care Prescription Renewal Team in Nashville, Tennessee. This role supports licensed healthcare team members in providing patient care, including direct patient care, educating patients and families, and following established care plans. The LPN will work collaboratively with Registered Nurses or other licensed healthcare professionals to evaluate patient progress. All patient care activities are performed under the delegation and supervision of a licensed professional, adhering to VUMC standards and licensure regulations. The position operates Monday through Friday from 8:00 a.m. to 4:30 p.m.

Responsibilities

  • Collaborates with patients/families and health care providers in the delivery of health care services.
  • Partners with the healthcare team by following established plan of care to ensure patient progress for effective outcomes.
