About The Position

The LPN is responsible for managing and processing medication refill requests and medication prior authorizations in a safe, and compliant manner. This role supports providers by ensuring appropriate medication continuation, patient education, and adherence to clinical protocols, while maintaining high-quality patient care and regulatory compliance. LPN renders safe nursing care to patients. The LPN administers medications, performs venipuncture, and performs related duties. The LPN manages the Patient Assistance Program (PAP) as well.

Requirements

  • Graduate of an accredited LPN/LVN school of nursing.
  • Current CPR certification is required.
  • Current license as a Licensed Practical Nurse in the State of Louisiana.
  • Experience in collaboration with interdisciplinary healthcare teams.

  • Experience in ambulatory care setting preferred.

Responsibilities

  • Manages the Patient Assistance medication program that aids with affordable medications for the underserved population.
  • Provides timely feedback with patients in regards to medication refill needs.
  • Review and process medication refill requests and prior authorization requests received via phone, patient portal, pharmacy, and fax.
  • Verify patient information, medication history, allergies, and last visit date in the electronic health record (EHR).
  • Assess refill appropriateness based on established protocols, standing orders, and provider guidance.
  • Identify refill requests requiring provider review and escalate as needed.
  • Ensure patients are up to date with required visits prior to refills.
  • Communicate with patient regarding refill status, medication instructions, and follow-up as needed.
  • Coordinate with pharmacies to clarify prescriptions and resolve discrepancies.
  • Document all refill activities accurately and timely in the EHR.
  • Initiate and follow through with prior authorizations and medication access issues as needed.
  • Perform medication reconciliation.
  • Provide patient education on medication use, side effects, and adherence.
  • Recognize and report adverse drug reactions or potential medication errors.
  • Responds to all emergency codes and drills and completes appropriate paperwork.
  • Follows Standard precautions by utilizing appropriate Personal Protective Equipment.
  • Enforces environment of Care policies (smoking, hand washing, trash disposal, etc.).
  • Maintains adaptability, supports teamwork, and resolves problems.
  • Identify patient eligible for medication assistance programs based on financial and clinical criteria.
  • Complete, submit and track PAP applications with pharmaceutical companies.
  • Maintain updated knowledge of available medication assistance programs and eligibility requirements.
  • Serve as a liaison between patients, providers, and pharmaceutical companies.
  • Ensure proper storage and handling of medications per regulatory guidelines.
  • Document all patient interactions and PAP activities in the EHR.
  • Maintain accurate logs of medication shipments, inventory, and distribution.
  • Follows chain of command with problems and suggestions.
  • Accepts additional assignments willingly; and assists others as required to meet quality service and productivity expectations.
  • Must be willing and able to travel to other CareSouth locations.
  • Other duties as assigned on a non-routine basis.
