About The Position

As a Licensed Practical Nurse (LPN) at CarDon and Associates, you will provide high-quality patient care across shifts and assist residents with their daily activities, comfort, and well-being. You will collaborate with a multidisciplinary team to develop and implement individualized care plans, monitor residents’ health status, administer medications, and educate residents and their families. Your work will support residents’ health goals and quality of life.

Requirements

• Current and valid Licensed Practical Nurse (LPN) license in the state of Indiana.
• Basic Life Support (BLS) Certification.

Nice To Haves

• Experience in a long-term care or assisted living environment.
• Proficiency in electronic health record (EHR) systems.

Responsibilities

• Administer medications and treatments as prescribed by the healthcare provider.
• Monitor and record vital signs, and report changes in resident conditions to the nursing staff.
• Assist residents with daily living activities, including bathing, dressing, and grooming.
• Maintain accurate and up-to-date resident records and documentation.
• Collaborate with healthcare team members to develop and implement care plans.
• Provide education to residents and their families.
