About The Position

The Licensed Practical/Vocational Nurse provides direct patient care under the supervision of a registered nurse. The role follows the plan of care, provides treatments, and collaborates with team members to support positive patient care outcomes. The position is with River Valley Home Health and Hospice in Bullhead and is PRN.

Requirements

Complete a requisite practical (vocational) nursing program.
Hold an active LPN or LVN license in the State of Arizona.
Comply with accepted professional standards and practice.
Demonstrate good verbal and written communication and organizational skills.
Be able to drive and/or access transportation for agency-related travel.
Maintain a valid driver’s license and automobile insurance coverage, and have access to an automobile.
Possess and maintain current CPR certification.

Nice To Haves

Three years of nursing experience preferred.
Community/home health or medical/surgical experience preferred.

Responsibilities

Provide direct patient care as defined in the State’s Nurse Practice Act.
Implement the plan of care initiated by the registered nurse.
Provide accurate and timely documentation consistent with the plan of care.
Collect assessment data and make observations; report findings to and collaborate with registered nurses and physicians.
Participate in coordinating home health services and report identified needs for other disciplines to the registered nurse and/or Assistant Director of Nursing or Director of Nursing.
Use equipment and supplies effectively and efficiently.
Participate in personal and professional growth and development.
Perform other duties as assigned by the registered nurse.
Follow written and oral instructions.
Maintain patience, tact, a cheerful disposition, and enthusiasm.
Comply with all training requirements.
