About The Position

Concierge Home Care is hiring Home Health Licensed Practical Nurses (LPN). This role offers the opportunity to take control of your schedule and income, valuing your time, expertise, and independence. The position involves 1:1 patient care in the home with a flexible daytime schedule and pay-per-visit compensation model, offering additional earning opportunities. The coverage area includes Panama, FL 32405 and Bay & Gulf Counties, with territories designed to minimize drive time through clustered scheduling.

Requirements

• Active, unencumbered Florida Licensed Practical Nurse (LPN) license
• Valid driver’s license, reliable transportation, and auto insurance
• CPR certification

Responsibilities

• Provide skilled nursing care under the supervision of a Registered Nurse
• Perform patient assessments and assist with care planning
• Administer medications and treatments as ordered
• Educate patients and caregivers on disease management and care plans
• Monitor patient condition and report changes to the supervising RN
• Collaborate with physicians and interdisciplinary teams
• Complete documentation using streamlined, AI-assisted tools
