About The Position

Under the general direction of the Wellness Director, primary responsibilities include, but are not limited to providing assistance to the residents in maintaining their highest level of functional independence and psychosocial wellness. The LPN will assume responsibility for administering quality care to meet the total needs of residents, including medication administration, handling, and storage in accordance with state regulations and company policy. This role involves assisting in maintaining a neat and orderly medication cart and storage room, ensuring accurate medication counts and security. The LPN will use sound nursing judgment to evaluate changes in resident situations, report them to the Wellness Director and/or physician when appropriate, and observe changes in resident status, needs, or preferences. The position also includes offering comfort and support, reporting resident and staff concerns, assisting with vital signs and weight monitoring, and ensuring compliance with the American House philosophy and departmental policies and procedures.

Requirements

  • Graduate from an accredited School of Nursing
  • Minimum 18 years of age
  • Must pass a pre-employment background check, drug screen, TB test and physical
  • 2+ years of nursing experience preferably with experience in long-term care or geriatrics
  • Up to date first aid and CPR certification
  • Current Unencumbered LPN license in the state
  • Knowledge of the physiology and psychology of the older adults
  • Willingness to work evenings and weekends
  • Exceptional staff training and customer service skills
  • Compassionate and friendly demeanor

Nice To Haves

  • Experience with dementia residents preferred

Responsibilities

  • Administering quality care to meet the total needs of residents
  • Providing medication assistance for residents in accordance with state regulations and company policy including pouring, preparing, assisting with administration, and documentation
  • Assisting in maintaining the medication cart and storage room in a neat and orderly manner
  • Ensuring accurate medication counts and security of the medication cart and medication storage room for assigned shifts
  • Complying with company policies and procedures related to controlled substances, narcotic count and documentation
  • Using sound nursing judgment to evaluate changes in resident situations and report to Wellness Director and/or physician when appropriate
  • Observing changes in Resident status, needs, or preferences and communicating them to the Wellness Director
  • Offering comfort and support, emotionally and physically, to residents
  • Reporting resident and staff concerns and/or issues to the Wellness Director and/or Executive Director, as designated
  • Assisting with vital signs and weight monitoring monthly or more frequently as required for all residents and completing all required documentation
  • Assisting with resident move-in and move-outs and with resident discharge planning as needed
  • Ensuring compliance to the American House philosophy as well as all written policies and procedures that govern the operation of the Resident Services Department
