About The Position

The Licensed Practical Nurse provides nursing care for assigned patients in assigned areas under the supervision of an RN, using evidence-based nursing as the standard. The role is based at Masonicare Health Center in Wallingford, Connecticut, and is scheduled for a day shift, 24 hours per week, every other weekend.

Requirements

Graduate of an accredited school of nursing for practical nursing.
Current Connecticut state license to practice as a licensed practical nurse.

Nice To Haves

One or more years of clinical experience in a hospital or similar health care setting is preferred.

Responsibilities

Provide nursing care appropriate to residents’ and patients’ ages, needs, and abilities, including administering medications and treatments, teaching, and implementing the interdisciplinary care plan.
Contribute to care plan development by collecting and recording data on an ongoing, systematic basis.
Communicate and collaborate effectively with residents or patients, families, colleagues, and interdisciplinary team members to meet resident, patient, and unit needs.
Document accurate, pertinent, and comprehensive information promptly and concisely, as required by regulations and facility policies and procedures; competently use the electronic medical record.
Supervise care provided by CNAs to ensure compliance with the interdisciplinary care plan and resident profile.
Participate in educational programs, in-services, meetings, and workshops, and use current literature to enhance professional skills and knowledge; attend all annual mandatory in-services.
Participate in quality improvement programs as directed.
Follow standards of the Diligent Minimal Lift Program.
Perform other duties as required.
