Requirements

  • Graduate of an accredited LPN/nursing program
  • Active, unrestricted LPN license (Massachusetts)
  • Current BLS/CPR/AED certification
  • Strong English communication skills (written and verbal)
  • Knowledge of long-term care practices and regulations
  • Ability to work independently and supervise staff effectively

Responsibilities

  • Provide direct nursing care based on individualized care plans and physician orders
  • Supervise and support nursing assistants and care staff in daily operations
  • Administer medications and treatments safely, ensuring proper patient identification
  • Monitor patient conditions and maintain accurate clinical documentation
  • Transcribe and implement physician orders into medical records and care plans
  • Assist physicians during rounds and communicate updates effectively
  • Manage admissions, transfers, and discharges
  • Perform clinical procedures such as: Wound care, Catheterization, Tube feeding, Suctioning, Specimen collection
  • Record and monitor vital signs (temperature, pulse, respiration, blood pressure)
  • Maintain inventory of medications and medical supplies
  • Educate patients and families on care plans and health management
  • Participate in audits/surveys and ensure regulatory compliance
  • Maintain confidentiality in accordance with HIPAA standards
  • Support onboarding and orientation of new residents
  • Collaborate with interdisciplinary teams to ensure quality outcomes
