About The Position

This role involves providing safe and respectful nursing care to patients/residents in a long-term care setting. The Licensed Practical Nurse (LPN) will conduct assessments, administer medications, collaborate with the interdisciplinary team, and participate in care planning and discharge planning. The position also includes assisting with staff orientation, responding to incidents, and performing related clerical duties. The role operates within the framework of minimum Nurse-to-Patient Ratios (mNPR) and adheres to the company's mission and values, emphasizing person-centered, socially just, compassionate care, and Indigenous cultural safety.

Requirements

  • Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience
  • Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant
  • Current basic cardiac life support certification
  • Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service
  • Physical ability to carry out the duties of the position
  • Ability to apply decision making and analytical skills
  • Ability to role model conflict resolution strategies
  • Ability to prioritize and organize work
  • Ability to teach and identify patient/resident learning needs

Responsibilities

  • Provide safe and respectful nursing care to an assigned group of patients/residents, adhering to established policies, individualized care plans, and priorities.
  • Conduct thorough assessments, monitor signs/symptoms, and communicate with the RN/RPN as needed.
  • Prepare patients for procedures, transfers, and admissions, coordinating with the interdisciplinary team.
  • Administer medications per protocol, monitor effects, and maintain communication with the team.
  • Utilize evidence-based resources to stay updated on nursing practices.
  • Respond to incidents such as codes, injuries, spills, and alarms, following procedures.
  • Assist with staff orientation, demonstrating procedures and providing support.
  • Address patient/resident and family learning needs, provide educational materials, and make referrals.
  • Attend care planning meetings, provide input, and advocate for patients/residents.
  • Collaborate with the team to develop and evaluate individualized care plans.
  • Assist with discharge planning based on patient status and external supports.
  • Operate and clean equipment, report malfunctions, and handle hazardous materials.
  • Perform clerical duties such as answering phones, faxing, and maintaining records.
