Licensed Practical Nurse
About The Position
Pemi Baker Hospice & Home Health is seeking a Licensed Practical Nurse to join their team. The LPN will provide nursing services to Home Care & Hospice patients in their homes. This role involves collaboration with RNs to help patients achieve their goals while also allowing for independent work. The position offers the opportunity to utilize and develop LPN skills. Pemi-Baker Hospice & Home Health, established in 1967, is a local, independent, non-profit agency providing Home Care and Hospice Care to over 30 communities in central and northern New Hampshire. They are supported by over 50 employees and volunteers and assist clients with returning home after hospitalization, provide physical therapy, and support families with advanced illness.
Requirements
- Current LPN license
- Valid Driver's License and insured vehicle
- Ability to work independently
- Self-motivated and well-organized
- Willingness to travel in all types of weather conditions
Responsibilities
- Provide nursing services to Home Care & Hospice patients in their homes.
- Collaborate with RNs to help patients reach their goals.
- Work independently while providing patient care.
