About The Position

The Licensed Practical Nurse provides licensed nursing services to residents in accordance with applicable guidelines and regulations. The role supports resident health and safety through medication administration, assessments, care planning, clinical documentation, communication with physicians and families, and response to emergencies. The position is based at Gorham House and may be scheduled for a day, evening, or night shift.

Requirements

• Training from an accredited licensed or vocational nursing program.
• General nursing education or area of study.
• At least 1 year of experience in senior care or a related field.
• Current Licensed Practical Nurse license in the state of practice.
• CPR certification.
• Acceptable English proficiency to perform job duties, communicate with supervisors and coworkers, and respond to and communicate during emergencies.
• Basic computer skills.
• Ability to work as part of a team and handle multiple tasks safely and effectively.
• High tolerance for working under pressure and handling critical situations calmly and professionally.

Responsibilities

• Administer medications to residents in compliance with federal and state regulations and Senior Lifestyle Corporation guidelines.
• May admit new residents, including completing admission assessments and documentation.
• Complete acute and long-term care plans or service plans.
• May track clinical systems, such as wounds or infection control, under the director’s direction.
• Provide general consultation regarding health-related concerns within the community.
• Notify physicians and/or family members about changes in residents’ health and provide proper documentation.
• Respond quickly to emergencies.
• Maintain close communication with all departments and staff members.
• Provide educational meetings for residents and staff.
• Understand the role in the Safety & Disaster Plan.
• Maintain confidentiality of residents’ and prospective residents’ information.
• May complete discharges, assessments, and clinical documentation as needed.
