About The Position

American Senior Communities is hiring a Licensed Practical Nurse (LPN) at Community Nursing and Rehabilitation. The role offers full-time night shifts and part-time day and evening shifts. Nurses develop meaningful relationships with residents and staff by learning their life stories and use their nursing skills to promote residents’ physical and psychosocial well-being. The position calls for a supportive presence, teamwork, leadership, collaboration, and strong interpersonal communication to help deliver coordinated, excellent care. Requirements

• Hold a current and valid Licensed Practical Nurse license in Indiana.
• Have proficient medication management skills.
• Be able to conduct thorough assessments and accurately document changes in residents’ condition.
• Have a strong passion for geriatric nursing and a commitment to senior care excellence.
• Have excellent communication and interpersonal skills.
• Demonstrate the C.A.R.E. values of Compassion, Accountability, Relationships, and Excellence.

Responsibilities

• Create a comforting and engaging atmosphere for residents.
• Promote teamwork within the care team to meet residents’ needs.
• Collaborate with nursing and other facility teams to provide coordinated, comprehensive care.
• Conduct thorough resident assessments and accurately document changes in condition.
• Manage medications.
• Demonstrate the C.A.R.E. values of Compassion, Accountability, Relationships, and Excellence with residents, family members, customers, and staff.
