About The Position

Requirements

• Valid LPN license in the state of practice.

Responsibilities

• Provide nursing care to residents in accordance with established care plans.
• Administer medications and injections as prescribed by the physician.
• Monitor residents' vital signs and report any changes to the supervising nurse.
• Assist with daily living activities, including bathing, grooming, and meals.
• Maintain accurate and timely documentation of patient care and progress.
• Collaborate with healthcare team members to develop and implement individualized care plans.
