About The Position

The Licensed Practical Nurse provides direct patient care under the supervision of a registered nurse. The role follows the plan of care, provides treatments, and collaborates with team members to support positive patient care outcomes. The nurse also documents care, educates patients and caregivers, coordinates home health service needs with the registered nurse or Clinical Supervisor, and uses equipment and supplies safely and efficiently. Other job-related duties may be assigned.

Requirements

• Graduate of an accredited practical nurse or vocational nursing program.
• Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
• Possess and maintain valid CPR certification.
• Comply with accepted professional standards and practice.
• Demonstrate good verbal and written communication and organization skills.
• Have a valid driver's license and an automobile in good working order, insured in accordance with the organization's requirements.

Nice To Haves

• Three years of nursing experience preferred.
• Community/home health or medical/surgical experience preferred.

Responsibilities

• Provide direct patient care as defined in the state Nurse Practice Act.
• Implement the plan of care initiated by the registered nurse.
• Provide accurate and timely documentation consistent with the plan of care.
• Assess and educate patients and family members or caregivers about self-care techniques and information related to the diagnosis and plan of care.
• Participate in coordinating home health services and report identified needs for other disciplines to the registered nurse and/or Clinical Supervisor.
• Use and prepare equipment and supplies effectively and efficiently while adhering to aseptic technique.
• Participate in personal and professional growth and development.
• Perform other duties as assigned by the registered nurse.
