About The Position

The Home Health Licensed Practical Nurse (LPN), also referred to as a Licensed Vocational Nurse (LVN), works under the supervision of a Registered Nurse to provide skilled nursing care to patients in their homes. The role includes verifying the plan of care, coordinating with the RN and physician, monitoring patients, supporting patients and caregivers with self-care, and completing clinical documentation. The position uses both per-visit-point and hourly compensation, depending on the work performed. Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. Requirements

At least 1 year of LPN or LVN experience.
Current LPN license specific to the state or states assigned.
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.
Candidates must pass a drug test before beginning employment.

Responsibilities

Verify the plan of care with the RN before each visit.
Collaborate frequently with the physician and RN to ensure patient needs are met, and notify them as soon as possible when a patient's condition changes.
Teach patients and caregivers self-care techniques.
Check vital signs.
Review all medications and perform medication reconciliation at each visit.
Perform specialized procedures ordered by the physician, such as Foley catheter changes.
Prepare equipment and materials for treatments using aseptic technique.
Complete documentation in a timely and accurate manner.
