About The Position

Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. Our goal is to deliver quality home health services, in good faith, to those who entrust themselves to our care and to minister to the unique needs of each individual with respect, dignity, and compassion. Concordia Visiting Nurses is seeking a dedicated and personable Licensed Practical Nurse LPN to join our team and provide compassionate care to patients and families. Licensed Practical Nurse LPN are needed for patient care in and around the Mars, Wexford, Gibsonia areas allowing you to work close to your home.

Requirements

Graduate of state-approved school with course in pharmacology
Current license in the State of Pennsylvania
Reliable, independent means of transportation to make home visits
Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state

Responsibilities

Monitoring vital signs
Assisting with wound care and personal care
Educating patients and families
Maintaining medical records
Collaborating with other healthcare professionals
Provide emotional support and companionship, building genuine connections with patients
