About The Position

The Licensed Practical Nurse (LPN) contributes to the nursing process of assessment, planning, implementation, and evaluation as part of patient care. Under the supervision of registered nurses, the LPN provides direction and guidance in patient education and activities aligned with their education and demonstrated competencies. The role is with Wellpath, an organization focused on providing compassionate care to underserved and vulnerable patients. The position is for nights, and an incentive bonus of up to $5,000 is available.

Requirements

• Graduate from an accredited school of nursing.
• Maintain current licensure as a Licensed Practical Nurse or Licensed Vocational Nurse in the state of employment.
• Be able to obtain and maintain CPR certification.

Nice To Haves

• A minimum of one year of clinic experience is preferred.

Responsibilities

• Under RN supervision, assess patients’ physical, psychological, and social dimensions in the Health Care Unit and housing units, contribute to individualized treatment planning, and consult with RNs and staff as needed.
• Implement treatment programs directed by Health Care Practitioners, administer medications (including IV therapy if certified), and perform diagnostic tests according to established protocols and procedures.
• Collaborate in controlled-substance counts at the beginning of each shift and follow safety techniques, procedures, and standard precautions when responding to codes or health emergencies.
• Assist with medical or minor surgical procedures; implement nutrition and therapeutic diet plans; and document nursing encounters using SOAP charting.
• Communicate effectively with nursing staff, physicians, and other relevant personnel.
• Attend mandatory staff meetings and training sessions, and follow policies and procedures while providing patient care.
