About The Position

The State of Connecticut Department of Mental Health and Addiction Services (DMHAS) is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join the Western Connecticut Mental Health Network (WCMHN) within Behavioral Health Home (BHH) services in Torrington, CT. This position is crucial for supporting individuals on their path to recovery through integrated, person-centered care. The role involves providing nursing care within the LPN scope of practice, administering medications, documenting client contact, implementing recovery plan interventions, and maintaining a safe therapeutic environment. The LPN will also co-lead psychoeducation groups, collaborate with behavioral health and medical staff, and assist individuals in accessing various services. DMHAS aims to promote the health and wellness of individuals with behavioral health needs through a comprehensive network of services.

Requirements

License or temporary permit to practice nursing in Connecticut by the State Board of Examiners for Nursing OR hold a Multistate or Compact Licensed Practical Nurse license.

Responsibilities

Provide nursing care for the Behavioral Health Home initiative within the LPN scope of practice.
Administer medications in accordance with nursing standards and DMHAS policies and procedures.
Provide services to individuals in recovery.
Document client contact and treatment to meet Joint Commission and agency standards.
Implement recovery plan interventions and maintain a safe therapeutic environment.
Co-lead psychoeducation groups and classes.
Collaborate with behavioral health and medical staff to ensure coordinated, holistic care.
Assist individuals in accessing services including health screenings, primary care, housing, transportation, and substance use treatment.
Deliver BHH services including comprehensive care management, care coordination, health promotion, transitional care, individual and family supports, and community resource referrals.
Render bedside care or mental health services to individuals with physical disabilities and/or an intellectual disability in instances where the services of a registered nurse are not required.
Assist doctors and nurses in the general care and treatment of individuals.
Measures doses and administers medications including insulin injections.
Documents medications using appropriate forms.
Performs narcotic counts.
Checks and may administer intravenous medications.
Reports changes in individuals to doctors.
Ensures that restraints are applied correctly.
Transcribes doctors medication orders.
Dispenses certain medications in doctor’s absence.
Applies topical medication.
Takes and charts temperatures, pulses and respirations.
Performs simple nursing treatments.
Monitors general individual needs including personal hygiene.
Reports on individual conditions and behavior.
Keeps the unit safe and healthy.
Administers enemas or suppositories.
Performs Foley catheterization care.
Orients individuals concerning medications and their use.
May perform emergency first aid.
May drive individual to necessary appointments/outings.
Performs related duties as required.
Performs the full range of duties as described in the class specification for the Mental Health Assistant 1 or Mental Health Assistant 2.
