About The Position

Adoration Home Health is seeking a passionate, dedicated Home Health Licensed Practical Nurse to provide expert, patient-centered care in Anmoore, West Virginia. The role provides one-to-one care in patients’ homes according to their plans of care, while supporting patient dignity and independence. The position offers flexible scheduling options, independent work with team support, and opportunities for advancement. The schedule is full-time, and a $10,000 sign-on bonus is available to external candidates only.

Requirements

• LPN/LPT/LVN degree from an accredited college of nursing.
• Current unrestricted registration and license in the state of practice and state of residence.
• Valid driver’s license, acceptable driving record, and proof of car insurance.
• Professional CPR certification.

Nice To Haves

• At least one year of nursing experience preferred.
• Home health care experience preferred.

Responsibilities

• Provide one-to-one care according to each client’s plan of care while maintaining the client’s dignity and independence.
• Administer medications, feedings, oxygen, and ostomy care as needed under the client’s plan of care, and assess results.
• Accurately document nursing care, communications, progress, and outcomes related to established goals.
• Inform the physician, case manager, and family of changes in a client’s medical condition or needs.
• Coordinate or order supplies, medications, and feedings to maintain adequate amounts on hand.
• Ensure patient needs are met in accordance with physician instructions.
