About The Position

Adoration Home Health is seeking a dedicated Licensed Practical Nurse to provide patient-centered home health care in Anmoore, West Virginia. The role provides one-to-one care in patients’ homes, following each patient’s plan of care while supporting dignity and independence. The position is full-time and offers flexible scheduling options, independent work with team support, and a $10,000 sign-on bonus for external candidates only. The coverage area is Anmoore, WV.

Requirements

• LPN/LPT/LVN degree from an accredited college of nursing.
• Current unrestricted registration and license in the state of practice and state of residence.
• Valid driver’s license, acceptable driving record, and proof of car insurance.
• Professional CPR certification.

Nice To Haves

• At least one year of nursing experience.
• Home health care experience.

Responsibilities

• Provide one-to-one care in accordance with the client’s plan of care while maintaining the client’s dignity and independence.
• Administer medications, feedings, oxygen, and ostomy care as needed, and assess and report results.
• Document nursing care, communications, progress, and outcomes for established goals; inform the physician, case manager, and family of changes in the client’s medical condition and needs.
• Coordinate or order supplies, medications, and feedings to ensure adequate amounts are available.
• Ensure patient needs are met in accordance with physician instructions.
