About The Position

FamilyCare Health Centers is seeking a Licensed Practical Nurse (LPN) to provide clinical support for their Extended Hours at the St. Albans, Patrick Street, and Cross Lanes locations. This role involves assisting providers with direct patient care, including clinical duties and telephone care, as well as supporting medical staff with clerical tasks, data entry, referrals, and patient communication within the scope of practice.

Requirements

  • Hold an active license as an LPN in the state of West Virginia.
  • Excellent verbal and written communication skills.
  • Effective interpersonal communication skills.
  • Basic computer skills.

Responsibilities

  • Assist providers with direct patient care through clinical duties and telephone care.
  • Assist medical support staff with clerical duties, computer data entry, referrals, and procedures within scope of practice.
  • Perform routine and emergent/urgent telephone and walk-in triage and forward information appropriately.
  • Perform patient assessments, including vital signs, blood pressure, height, weight, BMI, pulse oximetry, pulse, chief complaint, and age-appropriate screenings.
  • Review, update, and reconcile allergies, medication list, problem list, reminders, family history, and surgical history.
  • Perform and complete quality management as applicable per patient, including self-management, goal setting, and follow-up.
  • Provide educational material for patients with chronic illness or in need of prevention education.
  • Clean and stock nurse's station, lab, patient bathrooms, equipment, and exam rooms daily.
  • Ensure exam rooms are cleaned and disinfected between every patient daily.
  • Attend and participate in staff meetings and requested committees.
  • Perform all on-site testing as ordered (or per standing orders) and document results into the EHR and bill.
  • Perform venipuncture, label tubes appropriately, and route all specimens to the correct lab.
  • Perform EKG, oxygen administration, nebulizer administration, pulse oximetry, cryo procedure assistance, and tympanogram.
  • Administer adult and childhood vaccines under the direction of the in-house provider and/or per approved standing orders.
  • Administer medications under the direction of the in-house provider and/or per approved standing orders.
  • Perform ordered screenings, including hearing, vision, and TB testing.
  • Obtain prior authorization for prescriptions, durable medical equipment, and other therapies as needed.
  • Assist providers with patient communication including lab and test result notifications, prescription call-ins, and patient call backs.
  • Administer wound care and dressing changes.
  • Ensure patient safety by monitoring and assessing the patient in urgent situations.
  • Perform additional duties as assigned.
