About The Position

Alvita Care is seeking compassionate, reliable, and professional Licensed Practical Nurses (LPNs) for per diem floor staffing in a facility setting in Patchogue, Suffolk County, New York. The role focuses on providing high-quality resident care and supporting resident safety and well-being. Most available shifts are 3 PM–11 PM on weekdays and weekends. Hours are flexible, and full-time or part-time hours may be available depending on facility staffing needs. Pay is $35–$40 per hour based on experience and assignment.

Requirements

• Valid New York State LPN license.
• Strong communication and documentation skills.
• Authorization to work in the United States.
• Updated medical and vaccination records, or willingness to obtain them.
• Completion of required drug screening and health documentation.
• Successful completion of Alvita Care's onboarding process before starting an assignment.
• Ability to meet facility scheduling and staffing requirements.

Nice To Haves

• Previous nursing experience.
• Long-term care or facility experience.

Responsibilities

• Provide quality nursing care and support to residents.
• Administer medications and treatments within the scope of the LPN license.
• Monitor residents' conditions and report changes to appropriate healthcare professionals.
• Maintain accurate and timely documentation using the facility's charting system.
• Follow facility policies, procedures, and infection control protocols.
• Collaborate with other healthcare professionals to promote resident safety and well-being.
• Assist with other nursing duties as assigned within the scope of practice.
