Requirements

  • Graduate of an accredited Practical Nursing program or Registered Nurse Program.
  • Current, unrestricted LPN/LVN, or RN license in the state of practice.
  • Knowledge of long-term care regulations, resident rights, and skilled nursing facility standards.
  • Experience providing direct patient care to geriatric and long-term care populations.
  • Ability to administer medications, treatments, and physician-ordered therapies safely and accurately.
  • Skilled in monitoring residents' conditions and reporting changes to the interdisciplinary care team.
  • Experience with electronic health records (EHRs) and clinical documentation.
  • Knowledge of infection prevention and control practices.
  • Strong assessment, communication, and critical-thinking skills.
  • Ability to supervise and delegate tasks to Certified Nursing Assistants (CNAs) in accordance with state regulations.
  • Ability to work effectively with residents, families, physicians, and interdisciplinary team members.
  • Compassionate, resident-focused approach to care with a commitment to preserving dignity and quality of life.
  • Current CPR/BLS certification.

Responsibilities

  • Record a patient’s medical history accurately
  • Take and record measurements of blood pressure, temperature, heart rate etc.
  • Observe patients under treatment to identify progress, side-effects of medications etc.
  • Monitor patients’ condition including fluid intake and output and compose patient charts
  • Assist ailing patients in daily necessary activities such as eating, bathing etc.
  • Administer injections, prescribed medications, enemas etc., clean and dress wounds and assist with other basic medical care tasks
  • Collect samples for lab testing as assigned
  • Provide emotional and psychological support when needed
  • Communicate with patients’ family or friends to provide advice, comfort and release instructions
