About The Position
Tennessee Quality Care is hiring a Full-Time Licensed Practical Nurse (LPN) to provide one-on-one Home Health care to patients throughout Shelby County, TN and the Memphis area. This role offers a Monday-Friday schedule, competitive hourly pay, mileage reimbursement, and more independence in your nursing career. As a Home Health LPN, you will provide skilled nursing care directly in patients' homes, working closely with the Registered Nurse and interdisciplinary care team. Home Health allows for the opportunity to build meaningful relationships with patients while providing individualized care outside of a traditional facility setting, offering the independence to manage your day with the support of an experienced clinical team.
Requirements
- Current LPN/LVN license in Tennessee
- Graduate of an accredited practical or vocational nursing program
- Current CPR certification
- Reliable transportation
- Valid driver's license and current automobile insurance
Nice To Haves
- 3+ years of nursing experience preferred
- Home Health, Medical-Surgical, Community Health, or similar nursing experience preferred
Responsibilities
- Providing direct patient care according to the established plan of care
- Completing skilled nursing visits in patients' homes
- Administering treatments and medications as ordered
- Monitoring patients and reporting changes in condition to the RN
- Educating patients and families on medications, treatments, and self-care
- Completing accurate and timely clinical documentation
- Following infection-control and safety procedures
- Collaborating with the RN, physicians, therapists, and other members of the care team
- Supporting patients in managing chronic conditions and maintaining independence at home
